Programme
1. Programme Executive (Community Resource Engagement and Support Team- CREST)

Responsibilities:

  • Conduct/organize outreach to community to promote mental wellness and increase public awareness of mental health
  • Conduct induction talks for Go-To-Point
  • Conduct mental health screening for clients after community outreach
  • Identify seniors who are at-risk/with mental health conditions and connect them with the right service for early diagnosis and treatment
  • Provide basic emotional support and monitoring to clients or caregivers of persons with dementia or mental health conditions through regular home visitations
  • Provide service linkage and help clients and caregivers locate the right care and assistance within the healthcare and social services systems
  • Engage key community partners such as grassroots organisations, social service agencies to raise their mental health awareness, understand services provided by CREST and coordinate mental health training and education

Responsibilities:

  • Participate in team assessment
    • Conduct functional assessment
    • Participate in First Home Visit interviews, goal formulation, team, ITEP and ITER meetings (when necessary)
    • Conduct home and school visits (when necessary)
  • Conduct the School Placement Assessment
    • Administer a cognitive assessment (WPPSI-4) and interview caregivers using an adaptive behaviour scale (Vineland™-II)

    • Formulate a report based on findings for students applying for admission into a SPED school

  • Work closely with intervention team in areas of assessment, goal development, intervention strategies and progress monitoring in a trans-disciplinary setting.
  • Maintain documentation on clients’ progress.
  • Conduct in-service and parent workshop talks.

Requirements:

  • MUST possess Diploma or Degree in Occupational Therapy from an institution recognized by the Ministry of Health under the Allied Health Professions Act.
  • Have obtained Full Registration with Allied Health Professions Council
  • Preferably with 1 to 3 years of relevant working experience
